What it measures

A urinalysis checks a urine sample for signs of infection, kidney problems, and diabetes. A machine tests for things like sugar, protein, blood, and white blood cells. The automated version does not include a look under a microscope.

When it’s ordered

Burning or pain when you urinate, suspected bladder or kidney infection, known kidney disease or diabetes, and checks before some procedures.

Medicare coverage

Medicare judges coverage case by case under its medical-necessity rules. Medicare does not publish a national list of diagnoses for this test. The local Medicare contractor in your state may have its own rules.

Typical price

Medicare pays a set rate of $2.25 for this test (2026 fee schedule). When Medicare covers this test, you pay nothing. A lab can bill far more if you pay on your own or if Medicare denies the claim, so ask for the price first.

One thing to watch

If the lab adds a microscope exam, that is billed as a separate test. Ask the office which one is ordered.
